HOW TO APPLY FOR A GRANT:
1. Grant requests must be submitted in writing to the Foliage SERTOMA Club of Apopka on the requesting
organizations letterhead.
2. The request must be signed by the executive director, board chairman, or president.
3. The request must contain proof of “non” or not-for-profit” status; e.g.,501 (c)-3 or 501 (c)-3 designation.
4. Submit proof as an attachment, copy of original IRS determination letter indicating tax-exempt status.
5. Specify the amount requested and indicate what the funds will be used for.
6. Indicate what percentage of the organizations funds are used for administrative costs.
7. Indicate what percentage of the funds granted will be used for the stated purpose of the request.8. The request must show how the funds will benefit the Greater Orlando Area, specifically the Apopka Area.
9. Identify your point of contact and phone number, address, fax number and email address.
KEEP IN MIND THE FOLLOWING:
· Community need must be present and identified.
· Requesting groups must be tax-exempt (does not apply to individuals).
· Contributions cannot be given to churches or religious organizations for the purpose of religious advocacy.
· Organizations must be within our operating area.
